Ready to start the process of seeking care for a loved one?

Our staff will make this transition as easy as possible and answer any, and all of your questions.

We can admit patients directly from the Hospital, a Primary Care Physician Office, an Assisted Living Facility, or from the patient’s home. Referrals are accepted from the Hospital or Primary Care Physician. Once we have obtained the referral we determine if we can meet the patient’s financial and clinical needs.

If we can meet the needs of the patient we work with the Primary Care Physician and/or the Hospital to obtain any authorizations needed prior to admitting the patient. We would like to extend an invitation to anyone who may be interested in admitting or placing a loved one in our Rehabilitation Center to stop by for a tour. No appointment is necessary. WHAT TO EXPECT:

Once you have talked to a staff member and seen the facility, please come with your insurance information, reasons for needing care, and any other questions you may have for us.

You can complete the admissions paperwork on your own if requested ahead of time, but we recommend filling out the paperwork with our staff to answer any questions you have.

Once you are done with the admissions process your loved one will arrive, be given a tour of the facility, and meet their staff.

The admissions process can take anywhere from a few days to a couple weeks depending on your timeline of services needed.
